How would you approach a parent or caregiver about the need for a referral for developmental screening of their child? What do you need to consider before talking to a parent or caregiver?

I would give the parents a call and make sure to first state the progressions the child has made, then talk about the concerns I have and to schedule a meeting to discuss options for their child and advice. Things to consider are having plenty of observations of the concerns of the child and to be sensitive to the parents because hearing about children having red flags can be very hard.
